Occasionally the third and fourth sacraments (Communion and Confirmation) are celebrated together (often an adult). This medal shows one image on each side. First Holy Communion is when a person receives the Eucharistic bread for the first time. In Confirmation a person re-confirms their vows of fidelity to God taken by his or her parents and Godparents at Baptism. Both sacraments are usually celebrated in the spring.
